The School Health Program implemented by the school nurse includes health screening and referrals, immunization and health record maintenance, medication storage and administration, first aid treatment, case management for students with special health issues, and health instruction. The Fogelsville Health Room has 3 beds, a
restroom, and a private treatment room. First aid supplies and a few medications are stocked in the Health Room.
The school nurse keeps a few items of clothing for temporary use by students (please wash and promptly return clothing borrowed by your child).

professional evaluation is needed.

Life Threatening Allergies and other
Special Health Conditions |
| Several students in our school have life threatening allergies to foods (i.e. peanuts, tree nuts, shellfish, and
eggs) and bee stings. Several students have severe health reactions when they consume foods that contain gluten, lactase, or food colorings. In order to keep these students safe and healthy, please consult with your child's teacher before sending in food that will be consumed in the classroom such as daily snacks or special day food treats.
Consider non-food items such as pencils, erasers, stickers, sharing a craft, or a book donation when choosing to celebrate your child's birthday at school. Your thoughtfulness in this matter will be greatly appreciated.

| The following regulations have been established by the Pennsylvania Department of Health:
|
| A
physical examination is required upon original entry into school usually kindergarten or first grade; may also be required for newly enrolled older students.
A physical exam is also required for grades 6 and 11.
|
| A
dental examination is required upon original entry into school and again in third grade; may also be required for newly enrolled older students.
|
| All students entering school for the first time must be fully immunized as a condition of school attendance. The following immunizations are required:
|
| Diphtheria and Tetanus - Four or more properly spaced doses of DTP, DTaP, Td or DT, or any combination of the three with 1 dose administered on or after the fourth birthday.
|
| Polio - Three or more properly spaced doses of polio vaccine. (Inactivated Polio Vaccine or Oral Polio Vaccine).
|
| Measles (Rubella) - Two properly spaced doses of live attenuated measles containing vaccine (preferably (MMRII) with the first dose administered at 12 months of age or older, or measles immunity
proved by serological evidence determined by the hemagglutination inhibition (HI) test or any comparable test.
|
| German Measles (Rubella) - One dose of live attenuated rubella containing vaccine (preferably MMRII) administered at 12 months of age or older, or rubella immunity proved by serological evidence
determined by the hemagglutination inhibition (HI) test or any comparable test.
|
| Mumps - One dose of live attenuated mumps containing vaccine (preferably MMRII) administered at 12 months of age or older, or a physician diagnosis of mumps disease indicated by a written record
signed by a physician or his/her designee.
|
| Hepatitis B - Three properly spaced doses of hepatitis B vaccine. (Currently the medical recommendation is that the second dose of hepatitis B vaccine be separated by at least one month after the
first dose and the third dose is separated by at least 4 months after the first dose and at least 2 months after the second dose.)
|
| Varicella (Chicken Pox) - One dose of live attenuated varicella vaccine administered at 12 months of age or older, or chickenpox immunity proved by laboratory testing, or a written statement of history of chickenpox disease from a parent, guardian, or
physician.

Illness |
| The Parkland School District has established the following guidelines for sickness. Please report contagious illnesses to the school nurse. If a student becomes ill, he/she should stay home until fully recovered.
Students should not come to school with
the following symptoms:

Temperature of 100 degrees or higher
|
|
Vomiting
|
|
Diarrhea
|
|
Severe coughing
|
|
Suspicious skin rash
|
|
|
Medications |
| The Parkland School District has established the following medication guidelines to guarantee safe handling and administration of medications at school. When an illness is serious enough to warrant the administration of
medication, the parent or guardian should attempt to administer the medication before or after school hours. If it becomes necessary for a parent/guardian to send prescribed medication to school please follow these instructions:
The medication should be sent in the original container and must be labeled with the following:

The child’s name and grade
|
|
The name of the medication
|
|
The amount of medication to be taken
|
|
The time the child is to receive the medication
|
It is the parent’s responsibility to assure an adequate supply of medication is in the Health Room.
|
| Any changes in the dosage or time of administration require new forms to be completed by the parent/guardian and physician.
|
| If your child needs to have prescribed medication with him/her on any field trip, it is the parents’ responsibility to personally notify the school nurse before each scheduled field trip so that arrangements can be made to comply with your
request.
|
| A
written authorization from the parents indicating the name, dosage, time, and duration that the medication is to be administered should be kept on file in the Health Room. This form must be completed
no later than the second day that the child is receiving the medication in school.
|
| A
written authorization from the physician indicating the diagnosis, name of medication,
dosage, time schedule, duration, method of administration and possible side effects is required for all prescription medication.
|
| Parents are encouraged to deliver all medications to the Health Room. Parents are required to deliver controlled substances or medications such as syringes or Ritalin directly to the Health Room.
